
Welcome!
Your appointment will consist of an exam and consultation where we explain the treatment options and work with you to develop a treatment plan. Oftentimes the treatment can be done the same day as the consultation. However, some situations present as more complex and, in that case, will require an evaluation first and a separate appointment to provide treatment.
Each member of our team is very experienced and loves what they do, and we want to make you feel welcome and taken care of. You’ll be greeted by our front desk staff, who will answer any of your questions prior to your appointment. Once you’ve checked in at our front desk and completed any forms, you’ll be taken back to gather the initial information about your tooth and have a consultation with Dr. Hackbarth.
Please expect to provide the following information at your first visit to our office:
- A referral slip from your dentist and any x-rays or images of the tooth if applicable. We will take our own x-rays before the evaluation and during treatment of your tooth.
- A list of medications you are taking will be helpful for filling out your medical history.
- If you have dental insurance, bring the necessary insurance card.
IMPORTANT:
A parent or guardian must accompany all patients under 18.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.), or if you require medication prior to dental treatment (i.e. prophylactic antibiotics, etc.).
We always have time to answer questions or concerns, we will be happy to talk you through your treatment options, what you can expect to experience, and give you the instructions to support a speedy recovery.
